it was most likely an accident and he didn't double check, i've had this happen when the page laggs well posting the comment and it just keeps on a white screen you kinda have to refresh the page which can lead to a duplicate if the data had been sent out and it just wasn't reloading
atomic, i would suggest double checking and deleteing duplicates
