Clearly state your issue, and remember to not give out your password.
2. Don't try to login, as it will most likely lock up your account.
3. Find your prepaid card or whatever you used to purchase minecraft and enter the pin code. (For me it was the pin code on the back of the Minecraft prepaid card)
This verifies that it's your account and you have ownership.
